This notice explains how Tri-Town Revival Foundation handles information submitted through this website. It is intended to support transparency and responsible intake practices.

Information we collect

Website forms may collect names, email addresses, phone numbers, age or grade, general program interests, availability, skills, and the information a person chooses to include in a message. Initial website forms should not be used to send medical records, diagnoses, Social Security numbers, financial account information, or other highly sensitive information.

How information is used

Form processing

Website submissions are transmitted through Formspree and delivered to info@tritownrevival.org. Formspree acts as a third-party form-processing service and may process technical information needed to deliver and protect submissions.

Children and youth information

A parent or guardian should complete youth intake forms. The website collects only the information needed for initial contact and general program planning. More sensitive information, when genuinely necessary, should be requested later through an approved and more controlled process.

Sharing and sale

Tri-Town does not sell personal information. Information may be shared only with people or service providers who need it for the stated program or operational purpose, or when disclosure is required to address safety, legal, or compliance responsibilities.

Retention and protection

Records should be retained only as long as reasonably needed for the program, inquiry, reporting, safety, or legal purpose. Access should be limited to authorized leadership, staff, or designated program personnel. No online system can guarantee absolute security.
